FEES SCHEDULE:

DOT Physicals$ 30.00

PLANS WE PARTICIPATE WITH:

We participate in most group health plans.  Check with our staff for an up-to-date list.

These programs require us to file claims directly with the insurance companies.

PLANS WE DO NOT PARTICIPATE WITH:

It is our policy that payment be made at the time of service.  We accept:

• cash
• checks
• MasterCard, Visa

We will file your insurance claim and instruct the insurance carrier to reimburse you directly.  If you prefer, we can give you a complete insurance receipt at the time of check out.

QUESTIONS CONCERNING FEES:

Since each patient's medical problems differ, it is often impossible to accurately estimate the total cost of services before they are rendered.  The cost of care will depend upon the nature of the illness or injury.  It will be comparable to costs associated with other physicians' offices even though care is provided without an appointment and we are open for extended hours.  The fee for each visit increases as complexity of the medical problem increases.
